Yes, we provide Medical Directors, Physician Assistants, and Nurse Practitioners with multiple years’ experience in both Skilled Nursing and Assisted Living facilities.

Geriatric Consultants has all three on staff. Our physicians are trained and licensed in fields including Geriatrics, Hospice & Palliative Care, Family Medicine, Emergency Medicine, Osteopathic Medicine, and Psychiatry. Our Nurse Practitioners and Physician Assistants specialize and have experience in Geriatric medicine.

Many of our doctors serve as Medical Directors for Skilled Nursing, as well as Assisted Living Communities. In that role, they visit the communities where they are Medical Directors.

If there is a Medical Directorship established with the location, there is no charge to the facility for patient care visits. The visit is billed to the patient’s medical insurance. We do not charge the facility for a Nurse Practitioner or Physician Assistant who is dedicated to a facility. There is an additional charge for physician visits when not serving as Medical Director. All our doctors are available 24/7 to collaborate with our Nurse Practitioners and Physician Assistants as well as answer any questions from the facility regarding patient care.

Yes, during regular business hours the provider assigned to the facility is available for calls or text messages. Between the hours of 5pm - 7 am and weekends, we have an on-call system that will put the caller in touch with a provider, Nurse or Physician, for any consultations regarding our patients.

In most cases, yes. In addition, verbal orders can be given with follow-up by the assigned provider during scheduled visit days.

Geriatric Consultants has a CMS sanctioned Chronic Care Management program, (CCM) which any patient who has two or more chronic conditions may enroll in for better management of their conditions and have access to additional care management. In addition to an assigned Provider, this program includes a Clinical Assistant on a scheduled basis to monitor the patients in the CCM program and assist in follow up & referral, maintenance of the patient’s care, and communication with the resident, family, and staff regarding the resident’s needs and care. This program has been successful in many areas, including decreasing returns to the hospital and the number of medications. It focuses on person-centered care and includes regular communication between our clinical assistant and the patient’s family, thus reducing the calls from family to facility staff.

No. We can establish the CCM program in any type of facility dependent on our patient numbers to better manage the care of our patients/residents. The services are paid for by CMS.

Our providers develop their own schedules based on their patient numbers. We generally pay a higher percentage share of the reimbursement rate than the industry average. We work to reduce our providers’ travel time and replace it with more time for patient care. We are always looking for Part time and Full-time licensed providers.

Patient & Families

Yes, our visit and service fees are billed to the patient’s insurance, no matter the type of insurance, including Medicare and Medicaid. Any co-pay will be billed to the patient or responsible party.
